BY COMPLETING AND SUBMITTING THIS AGREEMENT FORM YOU ARE BINDING YOURSELF TO THE TERMS AND CONDITIONS OF OBTAINING A NEW YORK CITY VIRTUAL PRESENCE AND YOU ARE AUTHORIZING CHARGES TO BE MADE TO YOUR CREDIT CARD IN ACCORDANCE WITH THAT AGREEMENT.

PLEASE BE AWARE THAT INFORMATION YOU SEND IN THIS MANNER, INCLUDING YOUR CREDIT CARD INFORMATION, IS BEING DELIVERED VIA SECURED MEANS, BUT NO SECURITY SYSTEMS ARE FOOLPROOF.
